Federal investigators have determined that the pilot whose helicopter crashed while filming a reality television show two years ago likely caused the accident that killed him and two others.
The National Transportation Safety Board issued its probable cause report Monday in the February 2013 crash that occurred during filming of a military-themed reality show for the Discovery Channel.
